Varicocelectomy
Varicoceles are dilated veins in the scrotum that raise testicular temperature and impair sperm production. Microsurgical varicocelectomy at the subinguinal level has the highest success rate and lowest recurrence risk. Thai fertility hospitals offer the microsurgical approach, which is relevant if you have previously been offered only conventional ligation at home.
Vasectomy reversal
Vasovasostomy reconnects the vas deferens following a vasectomy. If the blockage is at the epididymis, a vasoepididymostomy is required -- a technically more demanding procedure. Surgeons at major Thai fertility hospitals perform both. Success depends on the interval since vasectomy and whether sperm motility has been affected in the intervening period.
Sperm retrieval
For men with no sperm in the ejaculate (azoospermia), sperm can be retrieved directly from the testis or epididymis. TESA uses needle aspiration; PESA retrieves from the epididymis. Micro-TESE is used for non-obstructive azoospermia, where sperm production is severely reduced and microsurgical dissection of testicular tissue is required to locate viable sperm. All retrieved sperm is used in ICSI.
Testicular biopsy
A diagnostic biopsy confirms whether sperm production is occurring and classifies the underlying cause of azoospermia. Results determine whether sperm retrieval is viable or whether donor sperm is the appropriate path forward.
How long should you plan for in Thailand?
- Varicocelectomy: allow five to seven days. The procedure itself is brief; most of the time is recovery observation and post-op review before clearance to fly.
- Vasectomy reversal: five to seven days, with same-day or next-morning discharge in most cases.
- TESA or PESA: three to five days, often timed to coincide with the partner's egg retrieval.
- Micro-TESE: five to seven days. Surgery takes two to four hours; andrology confirmation of viable sperm adds several more before a result is available.

Sperm parameters typically take two to three months to reflect changes, as spermatogenesis takes approximately 74 days. Your clinic will schedule a follow-up semen analysis at the appropriate interval.
What if no sperm is found during micro-TESE?
The surgical and andrology team will review findings with you before discharge. A second attempt is occasionally possible after a recovery interval; your surgeon will advise based on what the biopsy showed.
